All surveys completed will be kept private, and will only be used by LouHIE and its research partners, Noblis Inc., and University of Louisville, for purposes of researching and developing health record banking services for people and organizations in the Greater Louisville area.  No individually identifiable data gathered through focus groups, telephone research, or other means, will be shared with outside parties without permission.
