Medicaid and SafetyNet Survey

Welcome.  

This survey is for people and organizations from the Medicaid and SafetyNet community in the greater Louisville area, including the following categories,

  • Medicaid purchasers - funds and/or carries the financial risk for health benefits for a group of Medicaid recipients

  • Advocates for Medicaid recipients

  • Individual representatives on Medicaid or Safetynet boards or groups

  • Social Services agencies

  • Providers of healthcare services for Medicaid, SafetyNet or Uninsureds

  • Representatives of the uninsured population 

  • Other organizations/individuals involved in the Medicaid and SafetyNet community
