Public Health and Public Education Survey

Welcome.  

This survey is for people representing public health and public education organizations, departments, agencies or programs which provides services for consumers in the greater Louisville area (9 counties including Clark and Floyd in IN).  People representing the following types of organizations should complete this survey:

  • County or City Public Health Department

  • Public health program (run by State, district or county)

  • Health and wellness program

  • Health education and dissemination program

  • Other government based group or agency to improve the public's health

  • Other not-for-profit organization to improve the public's health
