Seniors and Medicare Survey

Welcome.  

This survey is for people representing departments, organizations, companies or government groups serving senior citizens (individuals age 55 or older) in the greater Louisville area in one of the following categories:

  • Medicare Plan Funder or Risk-Carrier

  • Medicare Program Manager

  • Medicare Supplement Plan Fund or Risk-Carrier

  • Government group or agency serving or supporting seniors related to their health

  • Senior Citizen Advocacy Group or Association

  • Other not-for-profit group or organization serving senior citizens

If you do not represent one of these categories, you can select a different survey.
